In the True Faith of a Christian, may God witness that I hereby vow and dedicate myself as a servant of Christ, the Poor, and those who have been changed by war.

I will adorn my service with true Charity, seeking to bring the reconciling love of Jesus to those who need it most.
I will wear on my person the Episcopal Church Service Cross, to constantly remind me of my faithful vow of always bearing in my heart the Cross of Jesus Christ, adorned with the virtues that attend it. So help me God.

In witness whereof I have prepared this document and signed it here at _____________________ on the ____ day of _________, in the year of our Lord _____.
